Obtaining the true ending in Blasphemous, known as the “canonical” ending, is a intricate and multi-step process that involves completing various specific tasks throughout the game. Follow this step-by-step guide to unlock the true ending:

    • Before the showdown with Esdras on the Bridge of the Three Calvaries, your first task is to find Perpetva’s grave in Echoes of Salt.
    • At the grave, the spirit of Perpetva will grant you the Incomplete Scalpular.
    • Esdras will then hand over Taranto to My Sister and depart to the Brotherhood of the Silent Sorrow.
  2. Esdras in the Brotherhood:
    • Revisit the Brotherhood and have a conversation with Esdras to obtain the Key Grown from Twisted Wood.
  3. Library and Holy Visage:
    • After helping Diosdado with his ghost problem, you can access a secret door in the library. This door leads to the Holy Visage, which is trapped in the roots of the Knot of Three Words.
    • Defeat the bosses Isidora Voice of the Dead and Sierpes, each holding an eye of the traitor.
    • Take the eyes back to the Holy Visage, who rewards you with the Apodictic Heart of Mea Culpa.
